 It could have happened due to a medical condition.  In general its not a good idea to talk about someone's appearance unless you are super close with them.

 How about, "Wow, you look great!" and leave it at that... I usually get a very good response with that.

I don't think it's rude. As someone who has gone through periods of weight loss, I appreciate it when people notice.

I really NEEDED someone, anyone, to notice and compliment me when I lost a lot of weight.

But no one did.

I have an older cousin, she says the same thing every time I see her, has my whole life, "you've lost weight!"

No matter what, she said it.

But when I actually did lose 80 pounds, she said nothing.

I'm not sure she was being mean or not all those other times, but it kinda feels like it.

When I lost weight no one said anything except a couple of old ladies and it was to ask if I had been sick. A coworker asked another coworker if I had lost weight on purpose. He didn't want to mention it to me in case it was because I was sick.

I had a co-worker and a cousin ask me if I lost weight on purpose. I had lost a tad too much weight and they wanted to make sure it was intended rather than from illness. I get that. But if the weight loss leaves you looking healthy rather than sickly, then I think noticing and complimenting them is a good idea.

Okay, and I totally mean it as a compliment! She does look great, but she is a co-worker so maybe I'll just let it alone.

I did have a really old guy at church ask me if I was pregnant once, that was embarrassing but I gave him a pass. I just said no I have a fat stomach lol

Instead of "you lost weight" try "you look great".
